To ensure prior assessment to consultation, enabling pertinent information, explanation and treatment throughout

• New clients must complete and return paperwork a minimum of 48hrs prior to appointment
• Confirmation for receipt of questionnaire from Simply Blended
• On Confirmation 50% of the treatment price paid through BACS
• Appointment confirmed
• Existing Clients pay 50% 48hrs prior to appointment in confirmation
• Balance of appointment by cash, cheque or through BACS
• appointments may be changed upto 24hrs prior to treatmemt

Nutritional Therapy and Integral Therapy paperwork include an additional 5 day food diary (4 week days, 1 weekend day/day off) for completion and this should be considered when booking an appointment
